It’s important to have a primary care physician who oversees your chronic condition. Mend Care is designed to be a bridge, providing support when you can’t get in to see your regular doctor right away or need a medication refill to hold you over. We work in conjunction with your primary care to make sure that there are no gaps in your care.
Here’s how we can help:
Flagstaff’s environment (altitude impacting COPD/CHF, seasonal changes, travel difficulties for appointments, lack of primary care providers) can make convenient in-home “bridge” care especially valuable.

Mend Care isn’t meant to replace your primary care doctor, who manages your chronic condition long-term. Think of us as a “bridge” between your regular appointments. We can help if you need a medication refill and can’t get in to see your doctor right away, or if you have a flare-up of symptoms and need a quick assessment. We can also communicate with your doctor’s office to keep them informed. We offer a more affordable and convenient way to get care when your doctor is not available.
We can refill many common medications for chronic conditions, such as those for high blood pressure, diabetes, or asthma on a short-term basis to ensure you don’t run out before your next primary care appointment. However, there are some restrictions. We do not refill controlled substances, and any refills will require a consultation with our medical team to make sure it is safe and appropriate. We will also communicate with your primary care regarding any refills we provide.
While we don’t order routine monitoring labs for long-term chronic condition management, we can perform certain lab tests if needed to assess a current concern or rule out other issues while you wait to see your primary care doctor. We also offer many wellness panels that can help give you a better picture of your overall health.
If your condition significantly worsens or you develop new, concerning symptoms, we recommend contacting your primary care doctor or seeking emergency care immediately. While we can provide temporary support and guidance, we are not a substitute for ongoing primary care or emergency services. We can, however, assess your condition and make recommendations on the best place to seek care.
